eZ-DI®Graded Plans - Benefits and main Policy Features

Many impaired risk people are declined by other carriers because of a pre-existing condition or high-risk occupation. eZ-DI solutions available through RISK insurance and reinsurance solutions. They are underwritten by Fidelity Security Life Insurance Company, Kansas City, MO. and are specifically designed for those cases.

Total Disability: Due to injury, sickness, nervous or mental disorder, the insured cannot perform the material and substantial duties of the insured's own occupation.

Issue ages: Age 18 through age 60 (last birthday) – 63 on some plans

Elimination Periods:– 30, 60, 90, 120, 180, 365, and 730 days (elimination periods vary by plan and state)

Benefit Periods: 1, 2, 3 and 5 years depending on plan. For disabilities commencing in the first policy year, 40% (33% on some plans) of the benefit will be paid for the benefit period. If the disability occurs in the 2nd policy year, 75% (66% on some plans) of the benefit will be paid. Thereafter, full benefits would be paid for the benefit period. The Monthly Benefit for Total Disability due to injury commencing in the first and subsequent policy years is 100% of the Selected Monthly Benefit for Platinum EZ Select plans. The Maximum Benefit Periods shown in the Schedule of Benefits will be reduced by 50% for a period of Disability which begins after the insured attains age 65.

Maximum Monthly Benefit: 60% of income up to $15,000 per month

Waiver of Premium: Waives the premium if Total Disability begins before the insured attains age 65. The premium is waived after 90 days of Total Disability, or after satisfaction of the Elimination Period, whichever is later.

Guaranteed Renewable To Age 65(Platinum Plus and Platinum EZ Select Plans): The insured's insurance under this policy is guaranteed renewable to age 65, as long as the insured pays the required premiums. Following the insured's attainment of age 65, premiums will increase each year on the anniversary date of the insured's coverage under this policy beginning with the anniversary date. As with all guaranteed renewable product, the company has the right to change the table of premium rates for all members of the insured's class on any premium due date on or after the First Policy Anniversary Date, but not more than once in any 12-month period.

Conditionally Renewable To Age 70(Platinum Plus and Platinum EZ Select Plans):The insured's insurance under this policy is conditionally renewable beginning with the anniversary date following the insured's attainment of age 65 to the first of the month following the insured's 70th birthday, provided that: 1) The insured pays the required premiums, subject to the grace period and 2) The insured continues to be actively at Work.

Presumptive Disability: If the insured suffers total and irreversible loss of speech and hearing, sight in both eyes, both feet, both hands, or one hand and one foot before age 70, the Monthly Benefit Amount is payable for a Total Disability due to sickness or injury up to the Maximum Benefit Period whether or not the insured is able to perform the material and substantial duties of the insured's own occupation. The Elimination Period does not apply to a Presumptive Disability.

Surviving Spouse Benefit: Upon death of the insured, a one-time payment equal to two times the amount of the last Monthly Benefit paid to the insured is provided to the surviving spouse. The insured must have been disabled for 180 days and receiving disability benefits.

Hospital Indemnity Benefit: After 30 continuous days of hospital confinement, this benefit provides $30 per day for up to 90 continuous days of confinement.

Accidental Death & Dismemberment Principal Sum Benefit: Up to $5,000 benefit paid should death or dismemberment of the insured occur within 90 days of a covered accidental injury.

Total Disability Benefit for Nervous or Mental Disorders: After the elimination period, insured will receive 50% of the Monthly Benefit Amount to a Maximum Benefit Period of six months for a Total Disability due to nervous or mental disorders. Benefits will begin after satisfaction of the Elimination Period as shown in the Schedule of Benefits. Payments will not be made at the same time for both Total Disability due to injury or sickness and nervous or mental disorders.

Optional Benefit Riders

Own Occupation Extension Rider: Extends the definition of total disability to own occupation for the full benefit period.

Hospital Indemnity Benefit Rider: If a hospitalization occurs because of a covered injury, sickness, nervous or mental disorder, additional benefit of $25 to $100 per day up to 365 days.

Partial Disability Benefit: Provides up to 50% of the Selected Monthly Benefit for Partial Disability, which begins before the Insured attains age 70. Benefits are payable for up to six months (reduces to three months at age 65), upon completion of the Elimination Period or immediately following a period of Total Disability. Partial Disability means that the insured is at work, but because of injury or sickness, is able to perform one or more, but not all, of the material and substantial duties of the insured's own occupation, or the insured is not able to be present at work for more than one-half of the time required to be actively at work (minimum 30 hours per week).
